Why This Training Matters

Falls Are the Leading Cause of Workplace Fatalities

When a worker falls, there is no second chance and no reaction time — a fall from 6 feet takes about six-tenths of a second. In 2024, 389 construction workers died from fall-related incidents (BLS), and fall protection has topped OSHA's most-cited standards list for over a decade. Every one of those citations, and nearly every one of those deaths, traces to the same gaps: no protection at height, the wrong equipment for the task, or gear that failed inspection nobody performed.

OSHA's current maximum penalties are $16,550 per serious violation and $165,514 per willful or repeat violation (effective January 15, 2025 — unchanged into 2026, as no annual inflation adjustment was issued). A single unprotected roof edge observed from the street is enough to open an inspection.

When is fall protection required?

  • 4 feet — general industry (non-construction) work

Low-Slope Roof Systems

Safety Monitors, Controlled Access Zones, and Warning Lines

On low-slope roofs — a slope of 4 inches in 12 or less — OSHA permits alternative systems where conventional fall protection is infeasible. A safety monitor is a competent person whose only job is to recognize and warn workers approaching a fall hazard. A controlled access zone restricts entry to only the workers performing the task, with access controlled by the safety monitor. A warning line system marks the safe work area: rope, wire, or chain rigged 34–39 inches high, flagged at least every 6 feet, on stanchions that withstand 16 pounds of lateral force before tipping, kept at least 6 feet back from the roof edge. Class A vs. Leading-Edge SRLs

Self-retracting lanyards work like a car seat belt — start to fall and they lock, arresting the fall within about 2 feet. But not every SRL is rated for roof and beam work. A Leading-Edge SRL is built so the lifeline is not cut or destroyed when it strikes the edge of a roof or steel beam during a fall. If your anchor is at foot level or your line can contact an edge, a standard Class A SRL is the wrong device — a distinction most training programs skip and ours tests.

Lanyards — Three Types, Three Uses

Anchor Points & Lifelines

Every anchor and every vertical lifeline must withstand 5,000 pounds, and the anchor belongs directly above the worker — never within 6 feet of the roof edge. Never work more than 4 feet to either side of your anchor: beyond that, a fall becomes a pendulum swing, and you strike the structure at nearly the same speed you would hit the ground.

Equipment Inspection — What Disqualifies Your Gear
  • Webbing: fraying, cuts, pulled webbing, loose stitching, UV discoloration from sun exposure
  • Buckles and D-rings: cracks, bends, sharp edges, rust — one bent metal part fails the harness
  • Label: must be present and legible, with last inspection date and service life
  • Rope grabs: cracks, corrosion, weak springs, missing rivets, locking mechanism function
  • Lifelines: broken fibers, fraying, thin sections; for cable — broken strands, cuts, crimping
  • After any fall: the harness is done. Never use a harness that has arrested a fall, and never store one in the sun

Rescue Protocol

Suspension Trauma and Rescue — the Part Most Courses Skip

A worker in a harness after a fall is NOT safe — suspension trauma can cause death within 20 minutes. Every worksite with fall protection must have a documented rescue plan.

⚠ What Is Suspension Trauma?

A worker hanging in a harness has roughly 20 minutes before suspension trauma becomes life-threatening: blood pools in the legs, return flow to the heart drops, and the worker passes out — shock can set in within 3 minutes.

After the rescue: harness off fast, victim in the seated "huddle" position — never laid flat, never allowed to stand, eat, or drink — kept warm, oxygen if available, and every victim goes to a medical facility even if they look fine.

Ladder Rescue
Needs more people than it sounds — two or more holding the ladder, several assisting the victim down. Ensure the ladder is in good condition and long enough before beginning. Have the victim support themselves if conscious.
Balcony / Window Rescue
Snag the lifeline with a boat hook. Cut and replace lines only with an OSHA-rated line already attached. A sharp knife must be available to cut webbing if needed — include a cutting instrument in every rescue kit.
Block and Tackle
Steady ascent, no jerking. Pad the line over sharp edges to prevent cutting the lifeline during the raise or lower.

Aerial Lift Rescue
Match the lift to the terrain — off-road conditions require a lift rated for unpaved surfaces. Brief the operator before beginning. Fall protection on every rescuer.

Who is required to receive fall protection training under OSHA?
Any worker who could be exposed to fall hazards must be trained. Under 29 CFR 1926.503 (construction), the employer must provide training to each employee who might be exposed to fall hazards. Under 29 CFR 1910.30 (general industry), training is required for each employee who could be exposed to fall hazards on walking-working surfaces. Training must be done before the worker begins work in the area where fall hazards exist.
How often does fall protection training need to be repeated?
OSHA does not specify a fixed retraining interval. However, retraining is required when: (1) the employer believes the worker lacks adequate understanding of fall protection requirements; (2) changes in the workplace make previous training obsolete; or (3) changes in fall protection systems or equipment make previous training obsolete. Many employers conduct annual refresher training as a best practice to stay ahead of OSHA scrutiny.
Can online training satisfy OSHA's fall protection training requirement?
Yes — online training can satisfy the knowledge-based components of OSHA's fall protection training requirement. OSHA has confirmed that online or computer-based training is acceptable as long as it covers all required topics, is interactive, and workers can ask questions. For equipment-specific training (e.g., SRLs or specific harness systems), hands-on demonstration is strongly recommended and may be required by some employers or general contractors. What is a "competent person" for fall protection, and do I need one?
OSHA defines a competent person as someone capable of identifying existing and predictable fall hazards and who has the authority to correct them. On construction sites, OSHA requires a competent person to supervise specific fall protection activities — including safety net installation, controlled access zones, and certain roof work. If you have workers on a construction site, you likely need a designated competent person. Aggie Safety provides competent person training and certification.
What happens when a worker is suspended in a harness after a fall?
Suspension trauma is a life-threatening emergency. A motionless worker suspended in a harness can lose consciousness and die within as little as 20 minutes as blood pools in the legs and the cardiovascular system is compromised. Call 911 immediately in every suspended-worker event. Encourage the worker to move their legs. Use a Suspension Trauma Strap (STS) if available. Your worksite must have a rescue plan in place before work begins — not improvised after a fall.
Can I reuse a harness after it has been in a fall?
A harness that has arrested a fall must be removed from service immediately — even if it shows no visible damage. The fall forces may have compromised the internal fiber structure of the webbing in ways that are not visible to the naked eye. Tag the harness "DO NOT USE," report it to your supervisor, and replace it. This applies to lanyards, shock absorbers, and SRLs as well — any component that has arrested a fall must be taken out of service.
At what height does OSHA require fall protection?
OSHA requires fall protection at 4 feet in general industry, 6 feet in construction, and 10 feet on scaffolds — and at any height when working over dangerous equipment, water, or impalement hazards. Boom lifts require tie-off at all heights, regardless of elevation.
